The ideal candidate brings 2+ years in behavioral health or related field, plus electronic documentation and MS Office proficiency.

Qualifications

2+ years experience in Behavioral Health, Social Services or equivalent field.
Willingness to travel 50-75% in Will, Kankakee and Grundy Counties, IL.
2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

Evaluate members’ needs/eligibility and recommend care resolution strategies.
Identify high-risk factors and coordinate appropriate referrals to case management.
Coordinate and monitor assigned care plan activities and progress.
Collaborate with clinicians and health programs to improve quality of care.
Escalate quality of care issues through established channels.
Negotiate options and services to meet members’ benefits and needs.
Engage members to promote healthy lifestyle changes and participation.
Mentor teammates and participate in regular team meetings.
Assist members in healthcare decision-making with providers.
Document care activities in line with regulatory and company policies.
