This full-time telework role requires 50-75% travel to Will, Kankakee and Grundy counties, with standard hours Mon-Fri 8-5 CST; travel is reimbursed.

You will engage with care teams, review member needs, document activities, and help members participate in healthcare decisions.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time in Will, Kankakee and Grundy Counties and surrounding areas, Illinois.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluation of Members: conduct evaluation of member's needs/eligibility and recommend an approach to case resolution.
• Identify high risk factors and service needs impacting outcomes and refer to appropriate teams.
• Coordinate and monitor care plan activities and progress.
