Job summary

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% in Winnebago, Boone, Stephenson, Ogle, Jo Daviess Counties, IL.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and inform care plans.
• Identify high risk factors and coordinate referrals to clinical teams.
• Coordinate and monitor care plan activities and progress.
• Collaborate with Medical Directors and other programs to improve outcomes.
• Escalate quality of care issues through established channels.
• Engage with members to promote healthy lifestyle choices and decision-making.
