Job summary

CVS Health is seeking a Case Management Coordinator to support members with Medicare/Medicaid in a remote-hybrid role focused on care coordination, benefits access, and education. You will collaborate with care teams to facilitate appointments, monitor care plans, and promote optimal health outcomes for diverse populations in Will County, Illinois.

The role requires 2+ years in behavioral health or related field, experience with electronic documentation and Microsoft Office, and willingness to

Qualifications

2+ years experience in behavioral health, social services or appropriate related field equivalent to program focus.
2+ years of experience of electronic documentation experience and Microsoft Office applications.
Bachelor's degree or non-licensed master level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling)

Responsibilities

Evaluate members' needs/eligibility and recommend a case resolution approach.
Coordinate and implement care plan activities and monitor progress.
Identify high risk factors and arrange appropriate referrals to clinical care.
Collaborate with case managers, supervisors and Medical Directors to address barriers to goals.
Engage with members to empower informed health decisions and active participation with providers.
