Qualifications

• 2+ years in Behavioral Health or related field.
• Willing to travel 50-75% around Southern Chicago, Illinois.
• 2+ years of electronic documentation experience and Microsoft Office proficiency.

Responsibilities

• Evaluate members and assess needs/eligibility; plan case resolution.
• Identify high risk factors and refer to clinical case management or crisis intervention.
• Coordinate and monitor progress of assigned care plans.
• Consult with Medical Directors and other programs to overcome barriers.
• Identify and escalate quality of care issues.
• Negotiate options to meet member benefits and healthcare needs.
• Engage members to promote healthy lifestyle changes.
• Provide coaching and empower independent medical decisions.
• Participate in team meetings and peer mentoring.
• Assist members in healthcare decision-making with providers.
• Document care in line with regulatory guidelines.
