myLaurel is hiring a Care Navigator, Level 1, to serve as the first connection for patients with the care team. This fully remote role supports scheduling, education, and ongoing care navigation from home.

Ideal candidates have healthcare call center experience, are comfortable with EHRs, and can work 12 PM–8 PM PST including weekends; strong communication and empathy are essential, with room to grow into senior roles.

Qualifications

• 2+ years of professional experience in a healthcare call center or patient-facing environment.
• Proficiency with Electronic Health Record (EHR) platforms such as Epic, Athena, Cerner.
• Private, HIPAA-compliant workspace with door.
• Reliable high-speed internet (≥20 Mbps down / 5 Mbps up).
• West Coast candidates preferred.
• HS diploma or GED required.
• Ability to work 12 PM–8 PM PST, including weekends.

Responsibilities

• Serve as lead contact for patients and caregivers via high-volume calls.
• Coordinate and schedule in-home and virtual care visits.
• Educate patients on myLaurel services and set clear expectations.
• Manage post-visit needs, including equipment, pharmacy requests, and documentation.
• Maintain accurate documentation in our EHR and internal systems.
• Collaborate with clinical and operations teams for a seamless patient experience.
• Work 12 PM to 8 PM PST, including weekends.
