Job summary

Ascend Medical is seeking a part-time Care Navigator to support patients through telehealth and in-clinic interactions. The role involves triaging, booking appointments, arranging referrals, and guiding patients through the care plan with emphasis on accessibility and patient-first service.

Ideal candidates have a Medical Assistant background, at least two years of MA experience, CPR certification, and familiarity with Athena EMR.

Qualifications

• High school diploma or equivalent required.
• Graduate from an accredited Medical Assistant program or equivalent healthcare preparation.
• CCMA certification preferred.
• Minimum of 2 years MA experience in healthcare.
• CPR certification current.
• Experience using Athena EMR preferred.

Responsibilities

• Book telehealth appointments, triage patients, and document interviews using EMR.
• Facilitate telehealth visits with pre-visit planning and equipment checks; orient patients to tech.
• Prepare patients for exams and arrange/coordinate referrals for routine tests.
• Coordinate with Care Coordinator to ensure care plans and follow-ups.
• Handle inbound/outbound communications (calls, chats, emails).
• Educate members on Ascend Medical services and insurance navigation.
