Job summary

Ascend Medical is seeking a Care Navigator to support new and existing patients across our hybrid and telehealth platforms. You will manage clinical and administrative tasks, triage patients, book appointments, and help patients understand their health plans.

The role emphasizes patient-first care, collaboration with the care team, and ensuring smooth navigation of medical services and insurance benefits.

Qualifications

• High school diploma or equivalent.
• Graduate from a nationally accredited Medical Assistant program or equivalent healthcare preparation.
• Certified Clinical Medical Assistant (CCMA) preferred.
• Minimum of 2 years MA experience in healthcare setting.
• Current CPR certification.

Responsibilities

• Book telehealth appointments and triage patients virtually.
• Prepare patients for examinations and coordinate referrals.
• Record patient interviews and histories in Ascend's EMR.
• Coordinate with Care Coordinator to ensure care plans and follow-ups.
• Manage inbound/outbound communications via calls, chats, and emails.
• Educate new members on Ascend Medical services and insurance navigation.
