Job summary

Ascend Medical is seeking a Care Navigator to support patients with clinical and administrative tasks in a patient-first primary care setting. You will handle triage, appointment scheduling, and education while working closely with the care team in a fast-paced environment.

The role emphasizes compassionate communication, organization, and the ability to guide members through Ascend’s services and insurance navigation, with a focus on delivering accessible care.

Qualifications

• Knowledge of clinical procedures, medical anatomy, ICD-10 and CPT codes.
• Excellent communication skills using medical terminology; professional conduct.
• Self-motivated, proactive problem solver with strong teamwork.

Responsibilities

• Book telehealth appointments and triage patients.
• Prepare patients for examinations and assist with referrals.
• Support care plans and follow-up appointments with providers.
• Manage inbound/outbound communications (calls, chats, emails).
• Educate new members on Ascend Medical services and benefits.
• Handle patient messages and route to appropriate team members.
• Provide service-oriented, compassionate patient interaction.
