Job summary

UnitedHealth Group seeks a Telephonic Care Navigator to support DSNP/CSNP members from a remote Colorado location. This full-time role requires working 9:00am–5:30pm local time, with overtime as needed, and begins with 4 weeks of training.

You will educate members on gaps in care, arrange appointments, link to community resources, and ensure access to PCPs while maintaining high service standards and HIPAA data security.

Qualifications

• High School Diploma or GED or equivalent work experience.
• Must be 18 years of age or older.
• 1+ years of call center or telephonic customer service experience.
• 1+ years of office setting experience using telephone and computer as primary tools.
• 1+ years of healthcare/insurance or social work/advocacy experience.
• Experience with Microsoft Word, Excel, and Outlook.
• Ability to work from home with high-speed broadband.
• Able to attend 4 weeks of training with no time off.
• Full-time availability, Monday–Friday, 9:00am–5:30pm local time, with possible overtime.

Responsibilities

• Completes telephonic outreach to DSNP/CSNP members using multiple outreach modes.
• Educates members on gaps in care and helps schedule provider appointments.
• Assists members with social determinants of health and links to resources.
• Ensures members have access to PCP and provider referrals.
• Outreaches members per program guidelines and meets quality metrics.
• Delivers excellent customer service to members and providers.
• Maintains schedule adherence and attendance; handles confidential data per HIPAA.
