UnitedHealth Group is hiring Telephonic Care Navigators to work remotely from Missouri. The role supports DSNP/CSNP members through telephonic outreach, care coordination, and resource linkage while meeting strict service metrics.

Full-time schedule, training for four weeks, and potential overtime. Candidates must have at least a high school diploma and relevant experience, including healthcare or insurance exposure, and be able to work 9:00am–5:30pm local time.

Qualifikationen

• High School Diploma / GED or equivalent work experience.
• 1+ years of call center or telephonic customer service experience.
• 1+ years in Healthcare / insurance or social work / community outreach.
• 1+ years of analyzing and solving customer problems.
• Must have access to high-speed broadband to work from home.

Aufgaben

• Makes telephonic outreach to DSNP/CSNP members via multiple outreach modes.
• Educates members on gaps in care and helps close gaps, including scheduling appointments.
• Assists with social determinants of health and connects to community resources.
• Ensures members have access to a PCP and maintains schedule adherence.
• Provides excellent customer service to members and providers.
• Maintains HIPAA-compliant handling of confidential health information.
