Job summary

UnitedHealth Group in Georgia is seeking a Telephonic Care Navigator to support DSNP/CSNP members through outreach, scheduling provider visits, and linking to community resources. This is a remote, full-time role with classroom and on-the-job training.

Role requires handling confidential health information, meeting performance metrics, and providing exceptional member service during 9:00 am–5:30 pm local time with occasional overtime. Training hours are 9:00 am–5:30 pm EST.

Qualifications

• High School Diploma or equivalent work experience is required.

Responsibilities

• Completes telephonic outreach to DSNP/CSNP members using multiple outreach modes including auto-dialer, manual and inbound calls.
• Educates member on gaps in care and assists with closure of gaps, including scheduling provider appointments.
• Assists members with social determinants of health and connects to community resources.
• Ensures member has access to PCP and maintains confidentiality per HIPAA and state/federal regulations.
• Meets quality and performance metrics and provides excellent customer service to members and providers.
