Qualifications

• Must meet one of the following: current, unrestricted independent licensure as a Social Worker; a 4-year or higher degree in a behavioral health field with 3+ years of direct experience; or an LPN with 5+ years of direct experience.
• 2+ years of experience in community health or healthcare setting
• 1+ year experience with local health providers or community support organizations addressing SDoH
• 1+ year experience with MS Office (Word, Excel, Outlook)
• Resident of Colorado
• Driver's license and transportation access

Responsibilities

• Engage members and families to complete comprehensive needs assessments (medical, behavioral, functional, cultural, SDoH)
• Develop and implement person-centered care plans aligned to readiness to change
• Collaborate with internal care team, providers, and community resources to execute care plans
• Provide referrals/linkages to Housing Navigator, Pharmacy Team, Peer Specialist, PCPs, and other providers
• Coordinate with community partners to support members with complex needs
