Job summary

RYACO is seeking a Remote Care Coordinator to deliver longitudinal Care Management services, engaging patients, reviewing charts and care plans, and coordinating with patients, caregivers, providers, and interdisciplinary team members in a fully remote setting.

The RCC may serve as primary Care Manager for an assigned in-office patient panel or as a Care Management partner supporting ALF settings; responsibilities include documentation, follow-up, and meeting productivity targets while

Qualifications

• Experience in healthcare, care coordination, patient support, population health.
• Strong verbal and written communication skills.
• Ability to independently manage a patient population and multiple priorities.

Responsibilities

• In-Office Care Management: primary Care Manager for an assigned patient panel.
• Identify and enroll eligible patients into Care Management programs.
• Obtain and document patient consent and complete comprehensive assessments.
• Develop and maintain individualized care plans and perform routine chart reviews.
• Conduct proactive patient outreach and ongoing monthly Care Management activities.
• Coordinate care and communicate updates with providers and interdisciplinary team.
