Job summary

CareVitality Inc. is seeking a Care Manager to oversee and coordinate comprehensive care management services for assigned patients, with remote work options from Texas, Illinois, Arkansas, Ohio or Florida.

The role involves creating and updating care plans, educating patients, supporting CCM and RPM programs, coordinating with providers and families, and documenting activities in the CareVitality platform.

Qualifications

• Must hold an active clinical license (RN/BSN/LCSW or equivalent).
• Bachelor’s degree in nursing, social work, psychology or related clinical field.
• Experience in care management or chronic disease management preferred.

Responsibilities

• Create, review, and update patient-centered care plans based on assessments and resources.
• Provide patients with written/electronic copies of care plans and document delivery.
• Support CCM, RPM, and other care management programs with education and resources.
• Identify patients with gaps in preventive services and assist with scheduling screenings/tests.
• Review allergy and medication profiles, monitor adherence and interactions, and communicate concerns.
