Job summary

CareVitality Inc. is seeking a Care Manager to oversee and coordinate comprehensive care management services for assigned patients, focusing on clinical oversight and patient-centered care planning.

This remote-friendly role supports CCM, RPM, and care coordination across multiple locations, requiring an active clinical license and strong documentation, communication, and time-management skills.

Qualifications

• Strong time management, focus, attention to detail, and communication skills.
• Ability to multi‑task across multiple systems including EHR and care management platforms.
• Proficiency with Microsoft Office applications (Outlook, Excel, Word, PowerPoint).
• Experience documenting time spent per patient and managing caseloads for CCM/RPM/BHI/PCM/TCM/APCM/RTM.
• Willingness to receive instruction, feedback, and guidance.

Responsibilities

• Create and update patient-centered care plans based on assessments across physical, mental, cognitive, psychosocial, functional, and environmental factors.
• Provide patients with care plans and document delivery in the medical record.
• Offer CCM education materials and resources for self-management and lifestyle improvement.
• Identify gaps in preventive health services and assist with scheduling screenings or tests.
• Review allergy and medication profiles, assess adherence, and communicate concerns to patients and providers.
• Engage patients through monthly care plan reviews to promote healthy lifestyles and reduce ED visits.
• Document all care management activities in the CareVitality platform and report time as required.
• Escalate patients needing reassessment per CareVitality protocols.
• Provide Transitional Care Management as needed; assist with transportation if required.
• Gather patient consent for care management and remote monitoring programs.
