Job summary

Ascend Medical in the United States is seeking a Remote Care Coordinator to deliver longitudinal Care Management services through proactive patient engagement, chart reviews, and care plan coordination. The RCC collaborates with patients, families, providers, and the interdisciplinary team from a fully remote setting.

The role may involve in-office primary care coordination or ALF support as assigned, with a focus on documentation, adherence to workflows, and meeting productivity expectations.

Qualifications

• Healthcare background (MA/LPN or similar) preferred.
• Experience in Care Management, CCM, APCM or population health.
• Experience in primary care or ambulatory healthcare settings.
• Experience with Athenahealth or ThoroughCare EHR platforms.
• Bilingual skills are a plus.

Responsibilities

• Provide longitudinal Care Management services with proactive patient engagement and documentation.
• Coordinate care with patients, caregivers, providers, and the care team.
• Maintain timely, compliant care plans and accurate documentation.
• Identify barriers to care and connect patients with appropriate resources.
• Support remote patient management across assigned populations.
