Job summary

CareVitality Inc. is seeking a Care Coordinator to support patients remotely by coordinating care services and helping them follow care plans. You will conduct regular outreach, identify gaps in care, and document activities in the CareVitality platform and EHR.

You will work closely with providers, care managers, and internal teams to ensure continuity of care and positive patient outcomes. The role may involve coordinating CCM, RPM, and related programs, with responsibilities including monthly

Qualifications

• Experience documenting time spent with each patient and managing caseloads in care management programs.
• Ability to work independently with supervision as needed and take initiative.
• Strong written and verbal English communication skills.

Responsibilities

• Conduct monthly follow-ups with patients based on comprehensive assessments and care plans.
• Provide CCM/RPM/BHI/PCM/TCM/APCM program education and resources to patients.
• Identify gaps in preventive services and assist with scheduling screenings or tests.
• Review and update medication profiles, assess adherence, and communicate concerns to patients/providers.
• Engage patients in monthly care plan reviews to promote healthy lifestyles and reduce unnecessary utilization.
• Document all care coordination activities in CareVitality platform and EHR.
• Escalate complex cases to Care Manager when needed.
• Provide transportation assistance if required.
• GATHERING patient consent for various care management and remote monitoring programs.
