Job summary

CenterLight Healthcare PACE is seeking a Care Manager to oversee overall care management and quality for participants. The role coordinates holistic care plans across disciplines and ensures culturally sensitive, client-centered services.

The ideal candidate has at least two years in care planning and administrative leadership within home health or related settings, with an active NYS license. Remote work is included with local travel as needed.

Qualifications

• Graduated from a Nursing education program acceptable to NYSED.
• Bachelor's degree in a related field preferred.
• Minimum of two years of care planning experience.
• Minimum of two years of administrative experience in a CHHA/LHCSA/acute care setting.
• Current New York State license and registration.

Responsibilities

• Participates in care planning meetings as needed.
• Reviews discipline-specific documentation for quality and addresses deficiencies.
• Coordinates a continuum of care with the IDT and PCPs.
• Coordinates long-term care services and community-based supports.
• Documents services per CLHC standards and regulations.
• Monitors care management activities and evaluates plan outcomes.
