Job summary

CenterLight Health System is seeking a Care Manager to oversee care management and quality of care for participants. The role involves coordinating holistic care plans and collaborating with an interdisciplinary team to meet health goals.

Required education is an NYSED-approved Occupational Therapy program, with 2+ years in a supervisory care-management capacity in home health or related settings. Schedule is 8:30AM–5:00PM, 37.5 hours weekly, with remote work within a 2-hour range.

Qualifications

• Education: NYSED-approved Occupational Therapy program.
• Minimum of 2 years administrative management experience in a certified home health agency, rehab, LHCSA, or related care setting.
• Customer Service experience required.
• Supervisory experience preferred.

Responsibilities

• Participates in care planning meetings and coordinates multidisciplinary input.
• Reviews discipline-specific documentation for quality and addresses deficiencies per policy.
• Monitors field staff documentation and addresses issues observed.
• Conducts coaching sessions with field staff as needed.
• Coordinates and authorizes services to meet member-specific needs and goals.
• Assists with long-term care services across sites and transitions between settings.
• Documents services per organizational standards and regulations.
• Evaluates plan of care effectiveness and makes adjustments as needed.
