Job summary

CareVitality Inc. seeks a Care Manager to oversee and coordinate comprehensive care management for assigned patients, providing clinical oversight, care plans, and documentation in line with regulatory standards.

The role is primarily remote, with occasional office reporting; requires an active clinical license (RN/BSN/LCSW) and a bachelor’s degree in nursing, social work, or a related field. Relevant care management experience is preferred.

Qualifications

• Active clinical license or credential (RN/BSN/LCSW or equivalent)
• Bachelor’s degree in nursing, social work, psychology, or related clinical field
• Experience or training in care management, CCM, RPM, BHI, PCM, TCM, APCM, or RTM preferred

Responsibilities

• Create, review, and update patient-centered care plans based on assessments and resource inventory
• Provide patients with care plan copies and document delivery in the medical record
• Educate CCM patients with health education materials and self-management resources
• Identify gaps in preventive services and schedule required screenings/tests
• Review and update allergy and medication profiles, monitor adherence
• Engage patients via monthly care plan reviews to reduce ED visits and readmissions
• Document activities in CareVitality Care Management platform and analyze trends
• Escalate patients needing reassessment per CareVitality protocols
• Provide Transitional Care Management services as needed
• Offer transportation assistance if needed
• Obtain patient consent for care management and remote monitoring programs
• Perform other duties as assigned
