CenterLight Healthcare PACE seeks a Care Delivery Manager to lead care delivery for older adults, ensuring coordinated, participant-centered services. You will oversee care plans, guide interdisciplinary teams, and drive quality outcomes while maintaining regulatory compliance.

Qualifikationen

• Active and unrestricted RN license in New York State.
• Prior leadership or management experience in healthcare, home care, PACE, or long-term care.
• Experience leading teams and driving quality outcomes.

• Oversee development and monitoring of individualized care plans.
• Coordinate Interdisciplinary Team (IDT) meetings and manage participant services.
• Monitor quality outcomes, regulatory compliance, and clinical documentation.
• Collaborate with physicians, nurses, social workers, therapists, and partners to support integrated care.
• Manage staffing, scheduling, and resource allocation for operations.
• Provide leadership and development to team members and promote accountability.
