Job summary

HumanEdge is seeking an experienced Care Manager – Social Worker to oversee care management services and ensure participant-centered care. In this role, you will review assessments, coordinate interdisciplinary care, and develop comprehensive care plans addressing medical, behavioral, psychosocial, and community needs.

This is a remote opportunity with orientation in the Bronx, NY. Ideal candidates are organized, collaborative, and compliant with regulatory and EMR standards.

Qualifications

• MSW from an accredited program.
• Current NYS LMSW or LCSW license.
• Minimum of 2 years of care management or case management experience.
• Experience coordinating comprehensive care plans across multiple disciplines.
• Strong knowledge of community resources and healthcare systems.
• Experience reviewing clinical assessments and care documentation.
• Excellent communication, organizational, and critical thinking skills.
• Proficiency with EMR and Microsoft Office.
• Ability to work independently in a remote environment while collaborating with an interdisciplinary team.

Responsibilities

• Manage participant care to ensure quality outcomes and continuity of services.
• Review assessments completed by field staff for clinical accuracy and completeness.
• Develop, implement, and update individualized, holistic care plans.
• Coordinate services with physicians, nurses, therapists, and community providers.
• Monitor participant progress and adjust care plans as needed.
• Identify barriers to care and coordinate appropriate interventions.
• Ensure documentation meets regulatory and organizational standards.
• Participate in interdisciplinary team meetings and case conferences.
• Promote participant safety, quality of life, and person-centered care.
