Job summary

TheKey is seeking a Geriatric Care Manager to provide holistic, client-centered management for aging at home. You will develop comprehensive care plans, reassess client needs, and coordinate medical and social resources to support safety, dignity, and independence.

Responsibilities include tracking client status, documenting changes, and advising families on medical decisions. Strong communication, problem solving, and MS Office/EHR skills are essential.

Qualifications

• 5+ years’ experience in community, hospital, hospice or home health case management.
• Bachelor’s or master’s degree in healthcare related field (preferred).
• Certified Care Manager (CMC) or Certified Case Manager (CCM).
• Knowledge of aging, dementia, and disabilities.

Responsibilities

• Develop Comprehensive Care Plans and implement them.
• Monitor and document client status, changes, and outcomes.
• Coordinate resources for client care and interact with other professionals.
• Maintain electronic health records including billing.
• Provide guidance to families for informed medical decisions.
