Qualifications

• Active, unrestricted RN, LCSW, or LCPC license in Illinois.
• 5+ years of professional experience in health care or care management.
• 2+ years of leadership experience.
• Ability to use EMR and other electronic information systems.
• Ability to travel across Illinois.

Responsibilities

• Lead teams of nurses, care coordinators and behavioral health professionals responsible for care management.
• Supervise, direct and evaluate a diverse care coordination team to ensure care coordination effectiveness.
• Develop system-view recommendations and reports to identify trends and actions.
• Interview, hire, and coach associates; onboard new staff and manage performance.
• Collaborate cross-functionally with leadership to align operations and improve metrics.
• Oversee discharge planning, post-discharge calls, and pre-admission assessments.
