Job summary

SCAN Health Plan seeks a Manager, Care Management to lead daily operations and quality improvement across care management programs. You will oversee care management pods, guide supervisors, and ensure high-quality, evidence-based care addressing medical, behavioral health, and social needs.

You will collaborate with clinical directors, develop dashboards, and promote performance improvement while maintaining privacy and regulatory compliance.

Qualifications

• Bachelor's Degree in Healthcare Administration, Nursing, Social Work, Public Health, Business Administration, or related field (or equivalent experience).
• 3+ years of progressive leadership experience in complex healthcare operations, care management, population health, or integrated care delivery programs.

Responsibilities

• Oversee daily operations and quality improvement of care management programs.
• Provide leadership across multiple care management pods and supervise managers.
• Develop dashboards and reports to monitor performance and ensure compliance with CMS/DMHC/DHCS standards.
• Collaborate with internal departments to align clinical and operational goals.
