Job summary

SCAN Health Plan is seeking a Director of Care Management to lead daily operations and quality improvement across care management programs, focusing on medical, behavioral health, and social needs of members. • Bachelor's Degree or equivalent experience in healthcare administration, nursing, social work, public health, business administration, or related field.

Responsibilities

• Manage daily operations and quality improvement of care management programs.
• Oversee care management pods, supervise managers, drive performance, ensure compliance with CMS/DMHC/DHCS/NCQA.
• Collaborate with other departments, develop dashboards/reports, and ensure privacy and confidentiality.
