Job summary

Tuesday Health is seeking a Care Coordinator for a remote role serving residents in the Canton/Akron area. The position supports scheduling for Complex Care Navigators, makes patient reminder calls, and helps coordinate care with health plan managers to ensure a seamless patient experience.

As a patient-facing role, you will foster relationships with patients through empathy, document all contacts, and assist with non-clinical tasks across interdisciplinary teams.

Qualifications

• High School Diploma / GED (or higher)
• Experience with escalated customer service issue resolution
• Knowledge of Microsoft Office products – Word, Outlook, PowerPoint – and proficiency in Excel (VLOOKUP, if)
• Phone/Fax system experience
• EMR experience

Responsibilities

• Support scheduling for Complex Care Navigators
• Make patient reminder calls
• Facilitate collaboration with health plan care managers
• Ensure proper documentation of all patient contacts
• Support outreach and navigation teams with operational and non-clinical tasks
• Answer patient calls and escalate issues with appropriate resources
• Identify and request necessary patient documentation from patients and providers
• Other duties may be assigned
