Job summary

Akron Children's Hospital in Ohio is seeking a Care Coordinator to serve as a liaison for behavioral health patients and families in ACHPS clinics. The role combines in-person support with remote care to coordinate services across the treatment team.

You will triage at-risk patients, participate in case conferences, monitor progress with evidence-based tools, and help navigate levels of care while addressing barriers to engagement and education.

Qualifications

• Education: High School diploma or equivalent required.
• 2–4 years of relevant healthcare experience; a Bachelor's degree can substitute.
• Proficiency in Electronic Health Records and MS Office is preferred.
• Excellent communication and organizational skills are essential.

Responsibilities

• Identify and triage patients at acute risk of suicide and arrange transportation to crisis services.
• Participate in regularly scheduled case conferences to coordinate and co-develop treatment plans with team members.
• Use evidence-based tools to monitor symptoms and maintain a registry tracking progress and identify patients not improving.
• Assist with navigation between levels of care within Behavioral Health (primary care BH, Specialty OP, PHP, IOP, PIRC).
• Work with family to arrange behavioral health appointments and address barriers to engagement, including motivational interviewing as needed.
• Provide education about behavioral health conditions and support medication treatment as indicated.
• Assess social determinants of health and coordinate with school health or community organizations as needed.
• Document in medical records.
• Work effectively with patients and families from diverse backgrounds.
• Other duties as assigned.
