Job summary

All Metro Health Care - NY is hiring an On-Call Care Coordinator Associate to ensure patient care needs are met efficiently. The role involves coordinating care, scheduling appointments, and communicating with patients, families, and health professionals.

You will handle inquiries, maintain records, and monitor the HHAeXchange dashboard with empathy and professionalism in a remote NY setting.

Qualifications

1+ years of customer service experience required.
1+ years of home healthcare experience preferred.
Bilingual preferred.

Responsibilities

Serve as the primary point of contact for patients, families, and healthcare professionals regarding care coordination.
Address inquiries, concerns, and provide guidance on accessing services and resources.
Schedule patient appointments, coordinating availability of healthcare professionals and patient preferences.
Monitor and update schedules to accommodate changes, cancellations, and rescheduling needs.
Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals.
Maintain accurate and up-to-date records of patient information and communications.
Monitor HHAeXchange dashboard between client/caregiver calls.
Provide compassionate support throughout the care coordination process.
