Valeris is seeking a Care Coordinator to support patients and healthcare providers across the patient journey. The role emphasizes relationship building, benefits navigation, and proactive communication within a remote-capable environment.

You will perform welcome calls, provide case management, and coordinate with nurse educators and payers. This position supports patients through coverage options, prior authorizations, and adherence programs while documenting activities in the Lynk system.

Qualifications

• Bachelor's degree strongly preferred or equivalent experience.
• Customer service and healthcare industry experience preferred but not required.

Responsibilities

• Build trusted relationships with patients and prescribers.
• Perform program welcome calls and benefit investigations.
• Manage inbound calls and triage patients to resources.
• Provide personalized case management to communicate coverage and next steps.
