Job summary

Oscar Health is seeking a Care Coordinator to join our remote Care Management team. You will provide comprehensive care coordination for members across CCM, RAP, and related programs, while acting as the primary point of contact for members and providers.

Responsibilities include handling inbound calls, coordinating post-discharge transportation, and supporting RN Care Managers to optimize health outcomes with cost efficiency and high confidentiality.

Qualifications

• 1+ years of experience with care coordination in healthcare.
• Proficient with EMRs (e.g., Medecision) and telephony systems (e.g., InContact).
• Certified Medical Assistant and familiarity with Health Insurance.

Responsibilities

• Be the primary bridge for members and providers in Care Management.
• Handle inbound calls and route via optimized IVR for one-call resolution.
• Coordinate care across CCM, RAP, and other programs while maintaining confidentiality.
• Manage non-emergent medical transportation (NEMT) via UberHealth per program guardrails.
• Escalate to Claims, Eligibility, and Care Navigation as needed.
• Lead special projects as subject matter experts.
• Collaborate cross-departmentally to enhance member experience.
