Job summary

Oscar Health is hiring a Care Coordinator to serve as the central entry point for the Care Management team. You will manage care coordination for members across CCM, RAP, and other programs, handling post-discharge transport and inquiries with a focus on one-call resolution.

This remote role is open to candidates in Arizona, Florida, Georgia, and Texas. Base pay ranges from $17.91 to $23.51 per hour, with benefits and 15 days of vacation per year.

Qualifications

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient with EMRs (e.g., Medecision) and telephony (e.g., InContact).
• Excellent interpersonal communication and listening skills.

Responsibilities

• Be the primary entry point for members and providers in Care Management.
• Handle inbound calls and route with one-call resolution.
• Coordinate care across CCM, RAP, TOC and various settings while maintaining confidentiality.
• Arrange non-emergent transportation via UberHealth following program rules.
• Escalate to Claims, Eligibility, and Care Navigation as needed.
• Lead special projects and collaborate cross-departmentally for member experience.
• Take on additional tasks as assigned.
