Job summary

Oscar Health hires a Case Manager to join its Complex Case Management team. The role collaborates across Oscar teams to deliver high-quality care and improve member health outcomes.

You will educate members, evaluate Social Determinants of Health (SDOH), and connect individuals with community resources while contributing to process improvement and pilot programs. This is a remote position with possible travel for events.

Qualifications

• 2+ years of clinical experience in hospital, outpatient or community care management.
• 2+ years working with patients with chronic conditions (e.g., diabetes, COPD, hypertension).
• 1+ years of Care Coordination and Navigation experience.
• RN licensure or MSW with relevant certifications as listed.

Responsibilities

• Coordinate care across inpatient, outpatient, ER, or home care with confidentiality.
• Partner with members and providers to navigate health challenges and create care plans.
• Assess SDOH and connect to community resources.
• Provide crisis intervention and advocate for patients to remove barriers to care.
• Educate members by phone, secure messaging, or in person on conditions and procedures.
• Ensure compliance with laws and regulations.
• Other responsibilities as assigned.
