Job summary

Oscar is hiring a Case Manager to join our Complex Case Management team. The Case Manager will educate members, evaluate Social Determinants of Health (SDOH), and connect individuals with community resources while contributing to process improvement initiatives.

This is a remote position open to multiple states; occasional travel may be required for meetings and events. A minimum of 2+ years clinical care management, 1+ years care coordination, and an active US RN license or MSW are required.

Qualifications

• 2+ years of clinical experience in care management (inpatient, outpatient, or community-based).
• 2+ years working with patients with chronic conditions (diabetes, COPD, hypertension).
• 1+ years in Care Coordination and Navigation.
• Active RN license (US) or compact license or MSW from CSWE-accredited program.

Responsibilities

• Coordinate care across settings while maintaining confidentiality and high standards.
• Partner with members and providers to create personalized care plans for better outcomes.
• Assess and address Social Determinants of Health by linking to community resources.
• Provide crisis intervention and act as member advocate to resolve barriers to care and adherence.
• Educate members by phone, secure messaging, or in-person on health conditions, discharges, and medications.
• Ensure compliance with applicable laws and regulations.
• Perform other responsibilities as required.
