Job summary

Oscar Health is seeking a Case Manager to join our team. The role collaborates across Oscar teams to deliver high-quality care and improve member health outcomes, educates members, evaluates Social Determinants of Health (SDOH), and connects individuals with community resources while contributing to pilot programs.

You will report to the Manager, Clinical Programs. This remote position is open to candidates in many states, including Georgia, with occasional travel for team meetings and events.

Qualifications

• 2+ years of clinical experience including hospital, outpatient or community-based care management
• 2+ years working directly with patients with chronic medical conditions (e.g., diabetes, COPD, hypertension)
• 1+ years experience in Care Coordination and Navigation
• RN licensure in the United States or active compact licensure; or MSW from an accredited program
• BSN; Licensed Master Social Worker or Licensed Clinical Social Worker
• CCM Certification and Complex Case Management certification

Responsibilities

• Coordinate care across inpatient, outpatient, ER and home care settings while maintaining confidentiality and professional standards
• Partner with members and providers to navigate health challenges and create personalized care plans
• Assess and address Social Determinants of Health (SDOH) by connecting members with community resources
• Provide crisis intervention and advocate for members to resolve barriers to care and treatment adherence
• Engage with members by phone, secure messaging, or in person to educate on health conditions, discharges, procedures, and new prescriptions
• Comply with applicable laws and regulations
• Perform other responsibilities as assigned
