Qualifications

• 2+ years of clinical experience in hospital, outpatient, or community care management.
• 2+ years of experience working directly with patients with chronic conditions (e.g., diabetes, COPD, hypertension).
• 1+ years of care coordination and navigation.
• Active, unrestricted RN licensure or MSW degree from a nationally accredited program.

Responsibilities

• Coordinate care across inpatient, outpatient, ER, or home care while maintaining confidentiality.
• Partner with members and providers to create personalized care plans.
• Assess and address Social Determinants of Health (SDOH) with community resources.
• Provide crisis intervention and advocate to resolve barriers to care.
• Educate members by phone, secure messaging, or in person about conditions and procedures.
• Ensure compliance with applicable laws and regulations.
• Perform other duties as assigned.
