Job summary

Oscar Health is hiring a Care Coordinator to serve as the central entry point for the Care Management department. You will coordinate care for members in CCM, RAP, and other programs, while enabling RN Care Managers to operate at the top of their license.

The role involves handling post-discharge transportation, inquiries, and cross-department collaboration to improve member health outcomes and reduce costs.

Qualifications

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient with EMRs (e.g., Medecision) and telephony systems (e.g., InContact).
• Excellent interpersonal communication and listening skills.

Responsibilities

• Be the primary bridge and entry point for members and providers with Care Management.
• Handle inbound calls and route with optimized IVR for one-call resolution.
• Coordinate care across CCM, TOC, and unassigned populations across multiple settings.
• Manage non-emergent medical transportation via UberHealth following program guardrails.
