Tuesday Health is seeking a Care Coordinator (CST) who is bilingual in Spanish to support interdisciplinary care teams and patient communications in a remote role limited to CST zones.

You will help schedule, remind patients, document contacts, and ensure smooth collaboration with health plan managers and providers, delivering a compassionate, patient-centered experience across home and facility visits.

Qualifikationen

• Bilingual in Spanish.
• High School Diploma / GED (or higher).
• Experience with escalated customer service issue resolution.
• Knowledge of Microsoft Office products (Word/Outlook) and Excel basics.
• Phone/Fax system experience.
• EMR experience.

Aufgaben

• Support scheduling for Complex Care Navigators.
• Make patient reminder calls.
• Facilitate collaboration with health plan care managers.
• Ensure proper documentation of all patient contacts.
• Support outreach and navigation teams with non-clinical tasks.
• Answer patient calls and transfer escalated issues to appropriate resources.
• Identify and request necessary patient documentation from patients and providers.
• Other duties may be assigned.
