Qualifications

• High School Diploma or GED (or higher).
• Bilingual in Spanish and able to communicate with diverse patients.
• Experience resolving escalated customer service issues.
• Proficient with Microsoft Office and basic Excel formulas (VLOOKUP, etc.).
• Phone/Fax system experience and EMR experience.

Responsibilities

• Support scheduling for Complex Care Navigators.
• Make patient reminder calls and coordinate with care teams.
• Facilitate collaboration with health plan managers and clinicians.
• Ensure documentation of all patient contacts.
• Assist outreach and navigation teams with non-clinical tasks.
