SCAN is a nonprofit health organization focused on helping older adults stay healthy and independent. The Care Coordinator will support SCAN members by assessing, planning, coordinating, and monitoring holistic needs to help them age in place.

The role emphasizes addressing care coordination needs, processing referrals, and navigating the healthcare system, with a focus on compassionate, data-driven care and collaboration with providers and community resources.

Qualifications

• 0–3 years working in healthcare or customer service.
• Use motivational interviewing techniques to address member concerns.
• Confidentiality and privacy of member information.
• Familiarity with CMS and DHCS regulations.
• Spanish strongly preferred: conversational, reading, writing.
• Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams).

Responsabilités

• Perform assessments and screenings to determine interventions and referrals.
• Act as triage and starting point of contact for benefits and access issues.
• Use motivational interviewing to understand member goals.
• Coordinate care using benefits and community resources.
• Document assessments and care plans in multiple systems within timeframes.
• Collaborate with county offices and governmental agencies as needed.
