Equality Health is seeking a Care Specialist to provide supportive care and assist diverse patient populations, identifying barriers to self-care and helping navigate the healthcare system. This remote, full-time role reports to the Supervisor, Clinical Outreach.

You will conduct health assessments via outbound calls, aid with scheduling, connect patients to resources, and collaborate with providers and care teams. A healthcare-related associate degree and 2+ years of experience are required.

Qualificações

• Associate’s degree in Healthcare or related field, or equivalent experience.
• Minimum two years of healthcare experience.
• Proficient with Microsoft Office and web-based tools.
• Strong outbound calling experience.
• Ability to work independently in a productivity-based environment.
• Excellent Excel basics.
• Reliable high-speed internet.

Responsabilidades

• Conduct outbound phone calls to members to conduct health assessments and identify care needs.
• Assist and support patients with scheduling appointments and accessing resources.
• Collaborate with patients, providers, referral coordinators and staff; document activities.
• Communicate available care coordination services to patients and caregivers.
• Screen for physical and behavioral health needs and refer as appropriate.
