Responsabilidades

• Provide phone triage and data entry for clinical information.
• Distribute reports and extract data for reports.
• Coordinate services per member's plan of care.
• Schedule HRAs, Level of Care Assessments, and care reviews.
• Support Care Coordinators and follow-up on equipment/transport.
