Sentara Health Plans is hiring a Care Coordination Technician for remote work in Virginia. The role supports care coordination staff and members through telephone triage, data entry, and report distribution.

You will schedule HRAs, Level of Care Assessments, and Plan of Care reviews, coordinating services and provider outreach to ensure timely care for members. The position requires a high school diploma, 2 years of clinical administrative experience, and familiarity with Medicaid/Medicare

Qualifications

• 2 years of administrative experience in a clinical setting.
• Knowledge of medical terminology and Medicaid/Medicare benefits.
• Familiar with Virginia regulatory compliance standards.
• Strong organizational and multitasking abilities; detail oriented.

Responsibilities

• Provide telephone triage and data entry for clinical and non-clinical information.
• Schedule Health Risk Assessments and Plan of Care reviews.
• Assist care coordinators in arranging services and provider outreach.
• Maintain accurate documentation and reports for LTSS leadership.
• Handle a high volume of member calls and coordinate follow-up as required.
