Qualifications

• Must possess a Health & Human Services degree or related field as listed.
• 1 year of Long Term Care experience required; LTSS/Medicaid exposure preferred.
• Experience in case management within a health plan is preferred.

Responsibilities

• Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
• Develops, participates in, and monitors multidisciplinary collaboration of services to patients where appropriate.
• Assists adult patients and their families with personal and environmental difficulties related to medical conditions up to end of life.
