Job summary

Sentara Health Plans is seeking a Care Coordinator for LTSS Waiver Members/Medicaid to work remotely in Northern Virginia (Fairfax, Springfield, Alexandria) with required in-person assessments and limited travel. The role focuses on case management for adult patients and families, coordinating multidisciplinary services in a Medicaid/ LTSS context.

Qualifications

• Must possess a degree in Health & Human Services or related field.
• 1 year of Long Term Care required experience.
• Case Management experience preferred.
• LTSS waiver/Medicaid/Health Plan experience preferred.
• Behavioral Health experience is secondary.

Responsibilities

• Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
• Develops, participates in, and monitors multidisciplinary collaboration of services to patients where appropriate.
• Assists adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including the time of terminal illnesses.
