Job summary

Sentara Health Plans is seeking a Care Coordinator in Winchester, VA. The role requires in-person assessments and telephonic coordination for members with Serious Mental Illnesses, Emotional Disturbances, and Substance Abuse Disorders.

The position is full-time (40 hours/week) with standard hours 8am–5pm EST, Monday–Friday. Travel up to 90 minutes each way may be required, and candidate must reside in NOVA (Winchester/Shenandoah).

Qualifications

• Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
• Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate.
• Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Sentara Health Plans is hiring a Care Coordinator in - NOVA (Winchester, Shenandoah)

Status: Full-time position (40 hours/week)

Standard working hours: 8am to 5pm EST, MF

Certification/Licensure:

• None required

Experience:

• Long Term Care- 1 year (required)
• Health Plan- 1 year; Medicaid- 1 year (preferred)
• At least one year of mental health experience (preferred)
• 1 year of experience with Children/Adolescents (required)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $24.91 - $40.07 hourly.
