Job summary

Sentara Health Plans is seeking a Care Coordinator for LTSS Waiver Members/Medicaid working remotely in Northern Virginia (Fairfax, Springfield, and Alexandria). The role includes case management and coordinating multidisciplinary care to support patients and families.

The position requires a health-related degree, at least 1 year of LTSS or Medicaid experience, and a strong emphasis on collaboration with care teams. Competitive benefits and full-time hours are provided.

Qualifications

• 1 year of Long Term Care required experience
• Case Management experience preferred
• LTSS waiver or Medicaid or Health Plan experience preferred
• Behavioral Health experience is secondary

Responsibilities

• Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate in, and monitor multidisciplinary collaboration of services to patients where appropriate.
