Sentara Health Plans in Northern Virginia seeks a Care Coordinator for LTSS Waiver Members/Medicaid. This remote role covers NOVA with in-person assessments in Culpeper and surrounding areas, traveling up to 1 hour.

It is a full-time position (40 hours) with standard hours 8 am–5 pm EST, M-F. Responsibilities include case management for patients and families, coordinating multidisciplinary care, and assisting with medical condition-related challenges up to end-of-life.

Qualifications

• Must possess a degree in Health & Human Services or a related field.
• Accredited degree per U.S. Education Dept. database.
• Experience in long-term care or Medicaid/health plans preferred.

Responsabilités

• Performs casework duties and provides case management to patients, families and caregivers.
• Develops and monitors multidisciplinary collaboration of services.
• Assists adult patients with personal and environmental difficulties related to medical conditions.
