Job summary

ARC is seeking a Care Coordinator Manager to lead clinical care coordination for Renew Health + Recovery's New England agencies. This remote, senior leadership role oversees CC leads, assigns tasks, and reviews weekly assessments to ensure high-quality SUD care.

The ideal candidate holds a master's degree (preferred), an active RN or LCSW license, and 2+ years of home-based care with 1 year of psychiatric experience. Competitive salary and benefits offered.

Qualifications

• Master's Degree preferred.
• Minimum of 2 years of home or community-based health care experience in the past 5 years and a minimum of 1-year psychiatric experience.
• Substance Use Disorder treatment knowledge preferred.
• Proficient in Microsoft Office Applications.
• RN, LCSW or state-specific equivalent with an active license to practice.

Responsibilities

• Supervision of Clinical Care Coordinator Leads/Clinical Care Coordinators across Renew's New England agencies.
• Assign and track Clinical Care Coordinator assignments.
• Review weekend assessments with IHSD weekly.
• Moderate and facilitate Case Review.
• Facilitate weekly Care Coordinator meetings.
• Identify training needs and discuss with Training Team.
• Attend SWAT meetings and assist with complicated cases.
